Infective Endocarditis in a Patient With a Transcatheter LOTUS Valve.

چکیده
A 71-year-old hypertensive man, who was an exsmoker with respiratory and renal comorbidities, presented with severe, symptomatic aortic stenosis. Due to his high surgical risk (EuroSCORE of 20.5%), we successfully implanted a LOTUS Boston Scientific percutaneous aortic valve transfemorally, with suitable depth. Implantation of a definitive pacemaker was also required, due to complete atrioventricular block. Two months later, the patient was admitted to hospital for acute pulmonary edema, and a new grade V/VI systolic murmur was auscultated in mitral focus. The transthoracic echocardiogram revealed a thickened and perforated anterior mitral leaflet, causing severe mitral regurgitation (Figures 1A and B). Transesophageal echocardiogram showed that the anterior mitral leaflet had heterogeneous echogenicity and irregular margins, suggestive of an abscess (video 1 of the supplementary material). The prosthetic aortic ring and its leaflets were thickened and had a mobile and filiform structure compatible with vegetation (Figure 2A and video 2 of the supplementary material). Moreover, vegetation was visible in the pacemaker lead (Figure 2B and video 3 of the supplementary material). Serial blood cultures isolated Streptococus gallolyticus. In view of the diagnosis of early infective endocarditis of the aortic valve complicated by severe mitral regurgitation due to valve rupture and pacemaker lead endocarditis, we discussed the case with the cardiac surgery team, who ruled out surgery due to the high risk. The patient died 48 hours later due to severe sepsis and heart failure. Clinical autopsy confirmed the diagnosis (Figure 3). This is the first known case of infective endocarditis in a patient with a LOTUS percutaneous aortic valve.

INFECTIVE ENDOCARDITIS IN CHILDREN
A total of 14 cases of infective endocarditis (IE) in children aged 6 months to 10 years were seen from December 1987 to December 1992 at the pediatric unit of Ayatollah Taleghani Medical Center. The majority of patients (12 of 14) were between 5 and 10 years of age. Acyanotic congenital heart disease was known to preexist in 78.6% and rheumatic valvular heart disease in 21.4% of cases. Or...
متن کاملTricuspid Valve Endocarditis in an Intravenous Drug Abuser: A Case Report
Recently, there have been more reports of cardiac valve infections due to the increasing number of intravenous (IV) drug abusers and chronic renal failure patients. Among cardiac valves, tricuspid valve is most commonly affected in the course of endocarditis. Appropriate management of these patients is a challenging and controversial issue in the field of cardiac surgery. Treatment options in s...
متن کاملInfective endocarditis mimics ANCA associated glomerulonephritis
Background: Sub-acute bacterial endocarditis (SBE) rarely presents with features of a small vessel vasculitis. Patients with SBE can also develop multiple serological abnormalities including ANCA. In this report, we present a case of infective endocarditis mimicked ANCA associated glomerulonephritis.Case presentation: A 57-year old male with a clinical picture of rapidly progressive renal failu...
متن کاملRight-sided Infective Endocarditis with Multiple Large Vegetations in a Case of Ventricular Septal Defect: A Case Report
Herein, we present the case of a 10-year-old child suffering from right-sided infective endocarditis with ventricular septal defect. Echocardiography revealed multiple rare large vegetations on the pulmonary valve extending into pulmonary artery along with a large vegetation over the septal leaflet of the tricuspid valve.
متن کاملInfective Endocarditis Associated with Transcatheter Aortic Valve Replacement: Potential Importance of Local Trauma for a Deadly Nidus
Recently, transcatheter aortic valve replacement (TAVR) has emerged as an alternative for the treatment of severe symptomatic aortic stenosis patients. Although experience with TAVR is increasing exponentially, few cases of post-TAVR endocarditis are reported. We present a case of 76-year-old man with infective endocarditis after TAVR who was definitely diagnosed by echocardiography.
